This outlines the framework for all consultations & packages with Melanie Smith
I will go through this with you in our first session together. Please talk to me if there is anything you would like clarified.
We will discuss and agree what support you want from me in your first session, and review your progress as we go along.
Payments can be made by bank transfer, a credit card or debit card. You shall be invoiced for payment before the consultation/ package starts, and this should be paid to confirm your appointment.
You are required to pay for sessions, which are missed or cancelled, with less than 48 hours notice. It is important that you attend sessions regularly. However, if you do need to cancel an appointment please let me know with at least 48 hours notice. You can do this by emailing me at firstname.lastname@example.org
Everything we discuss in our sessions will be confidential as well as the fact you are a client of mine. My client database, as well as any notes I might take are kept on a password-protected computer. Any paperwork I have is kept securely and shredded at the end of our contract.
As I can work on Zoom, it is best that you have a working internet connection.

I am responsible for contacting my GP about any health concerns.
If I am receiving treatment from my GP, or any other medical provider, I should tell him/her about any nutritional strategy provided by Melanie Smith. This is necessary because of any possible reaction between medication and the nutritional programme.
It is important that I tell Melanie Smith about any medical diagnosis, medication, herbal medicine, or food supplements, I am taking as this may affect the nutritional programme.
If I am unclear about the agreed programme, I should contact Melanie Smith for clarification.
Zoom or face-to-face consultations will not be recorded.
We receive, collect and store any information you enter on our website or provide us in any other way. In addition, we collect the Internet protocol (IP) address used to connect your computer to the Internet; login; e-mail address; password; computer and connection information and purchase history. We may use software tools to measure and collect session information, including page response times, length of visits to certain pages, page interaction information, and methods used to browse away from the page.
